What are Droopy Eyelids – and How Do They Develop?

The term 'droopy eyelids' usually refers to an overhang of skin that partially covers the movable upper eyelid. However, several possible medical causes must be distinguished.

Excess Upper Eyelid Skin: Dermatochalasis

In dermatochalasis, the skin of the eyelid becomes slack and forms an overhang. Skin aging and UV exposure can contribute to this; individual anatomy also influences the appearance.

However, a barely visible eyelid crease does not automatically mean that there is skin laxity requiring treatment.

Drooping Eyebrows: Brow Ptosis

If an eyebrow sags, it can displace tissue over the upper eyelid. The eye area then appears more covered, although the cause is not exclusively in the eyelid skin.

Therefore, the forehead, eyebrows, and upper eyelids should be examined together. Depending on the extent, different forms of eyebrow lifting may be considered.

Drooping Eyelid Margin: True Ptosis

In ptosis, the upper eyelid margin itself is too low. This can be due to changes in the levator muscle or its nerve supply.

Several of these changes can occur simultaneously. Therefore, the term 'droopy eyelids' is not sufficient to select a treatment.

Removing Droopy Eyelids with Botox: Is it Possible?

Botox cannot remove excess upper eyelid skin. The active ingredient does not break down skin or fatty tissue. What may be possible is a limited change in eyebrow position through a so-called chemical eyebrow lift.

How an Eyebrow Lift with Botulinum Toxin Works

Lifting and lowering muscles are involved in the position of the eyebrows. If selected eyebrow-lowering muscles are relaxed, the lifting effect of the forehead muscles can become relatively stronger.

In a suitable initial situation, this can particularly lift the outer eyebrow slightly. The eye area can appear more open.

A systematic review of eleven studies with a total of 585 participants describes measurable changes in eyebrow position. However, the results differed significantly. A specific lifting height cannot be promised for individual persons based on this.

Wrinkles at the Sides of the Eyes

Crow's feet are caused by the repeated activity of the orbicularis oculi muscle. They are not to be equated with a droopy eyelid.

Who can benefit from a Browlift?

A chemical Browlift in Düsseldorf can be considered if a slight, muscularly influenced eyebrow descent contributes to the appearance.

Significant excess skin, pronounced brow ptosis, or true eyelid ptosis, however, require a different assessment. The crucial factor is not only how much the upper eyelid appears covered, but also which structure actually needs to be changed.

The effect is temporary. A Browlift with Botox in Cologne is therefore not a permanent alternative to every surgical procedure.

Which risks are particularly important for droopy eyelids?

Bruising, headaches, asymmetries, and an undesirable lowering of the eyebrow or upper eyelid are possible. An unfavorable weakening of the lifting forehead muscles can worsen the appearance. Existing eyelid and eyebrow drooping must therefore be considered before treatment.

If the general skin quality, rather than excess skin, is the primary concern, special skin boosters can be considered.

Do Creams, Home Remedies, or Facial Exercises Help?

Here, a distinction should be made between skin care, temporary changes, and anatomical correction:

  • Eye creams: Can nourish dry skin and improve skin feel. However, this does not mean that excess skin is removed or a drooping eyelid margin is lifted.
  • Home remedies: A temporarily swollen eyelid is not automatically a droopy eyelid. Recurring or persistent swelling should be investigated.
  • Facial exercises: A 2024 overview reports improvements in general signs of aging but calls for more robust studies. It cannot be inferred from this that exercises permanently eliminate excess upper eyelid skin. Systematic review of facial exercises
  • Eyelid tapes: Can temporarily cosmetically change the eyelid crease. However, they do not treat the cause. In case of skin irritation, they should not be used further.

A short-term altered appearance is therefore not equivalent to a permanent removal of droopy eyelids.

When is Surgical Upper Eyelid Lift Useful?

An upper eyelid lift, also called blepharoplasty, is particularly considered when excess skin actually needs to be removed. Depending on the findings, fatty tissue is also treated.

Whether the eyebrow position or the levator muscle also needs to be corrected is a separate question. Simple skin removal does not automatically solve every cause of a drooping upper eyelid.

Healing Phase and Durability

Swelling and bruising may occur after surgery. About two weeks are often planned for visible recovery; complete healing can take longer.

The result can last long-term, but it does not stop natural aging. Therefore, a permanently unchanged eyelid shape cannot be guaranteed.

Possible Risks

Risks include noticeable scars, asymmetries, eye irritations, bleeding, and infections. Serious complications with impaired vision are rarely possible.
