Botox Aftercare: What Not to Do After Botox?

After a Botox treatment, you should not vigorously rub, press, or massage the treated areas. Many doctors also recommend staying upright for about four hours and avoiding intense exercise, saunas, sunbeds, and alcohol on the day of treatment.

These time recommendations are mostly cautious suggestions. For many traditional Botox rules, there is only limited direct study data. Therefore, if you accidentally lay down briefly or drank a cup of coffee, it doesn't automatically mean that the result will be worse.

Crucial are the individual instructions from your treating physician. They always take precedence over general recommendations from the internet.

Which rules after Botox are really important?

The most important aftercare rule is: Do not apply unnecessary pressure to the treated areas immediately after the injection. Vigorous rubbing or massaging can also irritate the small injection sites and increase redness or bruising.

Many other recommendations – such as staying upright for several hours or a longer sports ban – are mainly based on caution and clinical experience.

A retrospective study published in 2025 involving more than 5,000 patients questioned the necessity of extensive aftercare rules. This does not mean that medical advice should be ignored. However, it shows that an accidental violation of the rules is usually no cause for panic.

How long should you not lie down after Botox?

Often, about four hours of sitting or standing upright is recommended. This so-called four-hour rule is intended to avoid unnecessary pressure on freshly treated areas.

However, there is a lack of clear scientific evidence that short periods of lying down alter the treatment result. The recommendation should therefore be understood as a cautious guideline and not as a hard medical limit.

If you accidentally lay down briefly, you should not massage the treated areas afterwards. Observe the development calmly and from this point onwards, adhere to your doctor's personal instructions again.

Can you sleep after Botox?

After the first few hours, you can usually sleep. You don't have to sit up all night or lie exclusively on your back.

If possible, you should avoid strong pressure on freshly treated facial zones during the first night. If you still wake up on your side or stomach, this alone is no reason to assume a bad result.

Can you bend over after Botox?

Bending over briefly, for example to tie your shoes, is unlikely to compromise the result. However, extended inverted positions and heavy lifting are often avoided as a precaution during the first few hours.

This includes, in particular, longer inversions during yoga or physical work where the head is held significantly downwards for an extended period.

How long should you avoid sports after Botox?

Many practices recommend avoiding intense sports for about 24 hours. Light exercise and walks are usually possible immediately.

Use clean hands and lukewarm water. Do not rub vigorously over the treated areas and initially avoid:

  • facial cleansing brushes,
  • aggressive peels,
  • massage devices,
  • Gua Sha,
  • vigorous rubbing with towels.

The water itself is not the problem. The main aim is to avoid unnecessary friction and pressure.

When can you apply make-up after Botox?

Make-up can usually be applied carefully after a few hours, if the injection sites are no longer bleeding and the skin is inconspicuous.

Use hygienically clean brushes or sponges and gently dab the product on. If there is redness, small wheals or irritated skin, it is better to wait until the next day.

Facial massage, peeling and cosmetic treatments

You should avoid vigorous facial massages immediately after a treatment. For more intensive cosmetic or apparatus-based procedures, a gap of several days to about two weeks may be advisable.

These include, for example:

  • microneedling,
  • radiofrequency treatments,
  • ultrasound treatments,
  • microdermabrasion,
  • intensive chemical peels,
  • lymphatic and facial massages.

The appropriate interval depends on the treatment area and procedure. If Botox is to be combined with hyaluronic acid, skinboosters or other injection treatments, the sequence must be medically planned.

Painkillers and blood thinners after Botox

Certain medications can increase the risk of bruising. These include, for example, anticoagulant medications and some anti-inflammatory painkillers.

Never stop taking prescription medications on your own. This applies in particular to ASA, Marcumar and other anticoagulants.

Inform the treating doctor about all medications, dietary supplements and relevant pre-existing conditions before the injection. Only the responsible medical professional can decide whether an intake may be adjusted.

What happens if a Botox rule is broken?

In most cases, nothing happens. Lying down briefly, bending over once, a cup of coffee, or light physical activity do not automatically lead to a malfunction.

The aftercare rules are intended to reduce avoidable risks. They are not a guarantee for a perfect result, and an accidental violation does not mean that the treatment has failed.

Inform the treating lounge if:

  • you have received strong pressure or a massage in the treated area,
  • unusual symptoms occur,
  • a significant asymmetry develops,
  • you are unsure due to another treatment.
  • Do not try to correct or massage the treated areas yourself.

What is normal after Botox?

Immediately after the injection, small bumps or wheals may be visible. They often disappear within a short time.

Other possible temporary reactions include:

  • slight redness,
  • small bruises,
  • pressure sensitivity,
  • a feeling of tightness,
  • light swelling,
  • headaches,
  • visible injection sites.

An initially uneven effect does not necessarily mean it's unusual. The treated muscles do not always react simultaneously.

Therefore, the result should not be assessed after three or four days. For a treatment of the forehead lines with Botox, the frown lines Botox or the crow's feet Botox, the development within the first two weeks is particularly relevant.

Which symptoms require medical clarification?

Contact the treating lounge promptly if an eyelid or eyebrow significantly droops, a pronounced asymmetry occurs, or symptoms worsen instead of lessen.

Immediate medical help is required for:

  • breathing difficulties,
  • swallowing difficulties,
  • unclear or altered speech,
  • pronounced general muscle weakness,
  • double vision or significant visual disturbances,
  • symptoms of a severe allergic reaction.

Such reactions are rare but can occur hours to weeks after treatment. In case of shortness of breath or significant swallowing difficulties, you should call emergency services.

Increasing redness, warmth, severe pain, pus, or fever can indicate an infection and should also be medically examined.

Aftercare for Forehead, Frown Lines, and Crow's Feet

For treatments in the upper face, the development of eyebrows and upper eyelids is central. A slight unevenness during the first few days can be an intermediate stage.

Do not unnecessarily press or massage the forehead and eye area on the day of treatment. Tightly fitting headbands, heavy protective glasses, or prolonged pressure from the hand should initially be avoided.

Which injection points are suitable for a natural result depends on the interaction of the forehead, eyebrow, and eye muscles. This is precisely why forehead lines, frown lines, and crow's feet are not treated according to an identical scheme.

What applies after Masseter Botox?

After a Masseter Botox treatment, you can usually speak, eat, and chew normally. If there is pressure sensitivity, it may be more comfortable to avoid very hard or particularly chewy foods on the day of treatment.

The relaxation of the chewing muscle develops gradually. A possible change in the lower facial contour takes significantly longer than the initial muscular effect because the muscle volume can only change gradually.

What applies after Botox for sweating?

For Botox for excessive sweating, it is not the mimic facial muscles that are treated, but the signal transmission to the sweat glands.

After treatment of the armpits, the skin may be temporarily sensitive. When you can use deodorant again, shave your armpits, or train intensely depends on the skin condition and medical recommendations.

Therefore, the aftercare rules for the face should not be applied unchanged to a hyperhidrosis treatment.

What applies after a Platysma treatment?

For a Botox treatment of the Platysma, particularly careful medical planning is required. The neck area is close to muscles involved in speaking and swallowing.

Therefore, strictly adhere to the personal aftercare instructions. Unusual changes in voice, swallowing difficulties, or breathing problems must be medically clarified immediately.

When does Botox start to work?

First changes can often be visible after a few days. The result then develops further and is usually assessed after about ten to fourteen days.

Before this time, individual muscle areas can relax at different speeds. An early asymmetry is therefore not automatically the final result.

If less effect than expected is visible after about two weeks, a medical check should first determine:

  • whether the initial musculature is particularly strong,
  • whether a deliberately cautious dosage was used,
  • whether all planned muscle areas react sufficiently,
  • whether the desired result is medically achievable.

Self-treatment or massage is not advisable.

Can Botox be dissolved again?

No. Botulinum toxin cannot be dissolved with an enzyme after injection. Its effect diminishes over time because the signal transmission between nerve and muscle is restored.

This distinguishes Botox from a hyaluronic acid filler. Hyaluron can, after medical examination and under certain conditions, be partially or completely degraded with a Hyalase treatment.
